XERF vs Ultherapy, Thermage, Sofwave, and RF Microneedling: An Honest Comparison

Five devices, five different mechanisms, and a lot of confusing marketing. Here is a straight comparison of XERF, Ultherapy, Thermage, Sofwave, and RF microneedling.

If you have spent any time researching skin tightening, you have run into the same wall everyone does. Five or six device names, all promising lift without surgery, all with before-and-after photos that look convincing, and no clear explanation of what actually separates them.

The names are not interchangeable. These devices use genuinely different physics, reach different depths, and suit different faces. Picking the wrong one is the most common reason people spend real money on skin tightening and come away underwhelmed.

Here is the comparison without the marketing language.

First, the Two Families

Almost everything in this category is either ultrasound or radiofrequency. Understanding the split makes the rest much easier.

Ultrasound devices focus sound energy to a precise point at a fixed depth, the same way a magnifying glass focuses light. The energy passes harmlessly through everything above and deposits heat exactly where it converges. This gives excellent precision, but the depth is largely fixed by the transducer you choose.

Radiofrequency devices pass an electrical current through tissue. Tissue resists that current, and resistance generates heat. Rather than focusing at one point, RF heats a volume of tissue. It is less pinpoint than ultrasound and more broadly distributed, and depth is controlled by frequency and by how the current is delivered.

Neither family is superior. They are different tools.

XERF

XERF is a monopolar radiofrequency device from Lutronic, part of Cynosure Lutronic, and it received FDA clearance in the United States in 2025. It is the newest option in this group, which is both its main advantage and the reason there is less long-term published data on it than on the older devices.

Two things distinguish it.

The first is dual frequency. XERF delivers two radiofrequency frequencies, 6.78 MHz and 2 MHz. Higher frequencies deposit energy more superficially and lower frequencies travel deeper, so running both means the device treats shallow, middle, and deep layers within the same session rather than requiring you to pick one target depth. Most RF devices in this category run a single frequency.

The second is the effect on the retaining ligaments. These are the fibrous bands that tether the deep tissue of the face to the overlying skin, and they are a large part of why a face holds its shape or does not. XERF is designed to heat and shorten these ligaments, which produces structural retraction rather than only surface firming. This is the mechanism the structural skin tightening name refers to, and it is the part that is genuinely distinct from older RF platforms.

Practically, it also has three depth settings, ten intensity levels, impedance feedback that reads the skin in real time and adjusts, and cryogen surface cooling so the epidermis stays protected while the depth heats.

Best suited to: mild to moderate laxity across the lower face, jawline, and neck, particularly in someone who wants meaningful structural change without needles or downtime.

Ultherapy

Ultherapy is micro-focused ultrasound and has been the reference standard for deep non-surgical lifting for well over a decade. It has the longest track record of anything here.

Its defining feature is depth. Ultherapy treats at 1.5 mm, 3 mm, and 4.5 mm, and that deepest setting reaches the SMAS, the fibromuscular layer that surgeons actually lift during a facelift. Nothing else in this comparison reliably reaches that layer.

It also includes ultrasound imaging, so the provider can visualize tissue before delivering energy.

The tradeoff is comfort. Ultherapy is consistently the most uncomfortable treatment in this group, because the energy is deposited in concentrated points deep in tissue. Most people describe it as tolerable but genuinely unpleasant, and it is usually done with some form of pain management. It also does relatively little for surface texture and tone, since it works below that level.

Best suited to: moderate laxity where real depth is required and the person is willing to trade comfort for it.

Thermage

Thermage is monopolar radiofrequency, the older sibling of the XERF technology, running at a single frequency of 6.78 MHz.

It heats a broad volume of the dermis and subdermal layer, producing overall contraction and tightening. It is well established, works over larger areas efficiently, and is a reasonable option for body treatment as well as face.

Compared with XERF, the limitation is that a single frequency means a single depth profile. You get broad tightening but not the layered targeting, and it does not have the retaining ligament mechanism. Newer generations improved comfort considerably over the original, but it is still generally described as warmer and more intense than XERF.

Best suited to: general laxity and skin quality across larger areas, including body.

Sofwave

Sofwave uses synchronized parallel ultrasound beams to create a thermal zone in the mid-dermis at 1.5 mm.

The deliberate choice here is safety through shallowness. By staying at 1.5 mm, Sofwave keeps its energy entirely within the dermis and well away from deeper structures, nerves, and fat. There is no risk of the fat atrophy that has occasionally been reported with deeper devices, and treatment is fast, usually well tolerated, and low risk across skin types.

The tradeoff follows directly from that choice. At 1.5 mm it does not reach the SMAS and it does not engage the deep structural layer. It improves skin quality, fine lines, and mild firmness very effectively. It is not a lifting device for someone with real laxity.

Best suited to: early laxity, fine lines, and skin quality in someone who wants low risk and no downtime.

RF Microneedling

RF microneedling is a different proposition entirely. Fine needles penetrate the skin and deliver radiofrequency energy from the needle tips at a controlled depth.

Because it combines mechanical needling injury with RF heat, it does two jobs at once. It remodels deeper tissue and it substantially improves surface texture, scarring, and pore appearance. Nothing else in this comparison addresses texture as well.

The cost is that it is genuinely invasive. The skin is punctured, there is real downtime of several days, there is some risk of post-inflammatory pigmentation on deeper skin tones if settings are aggressive, and the treatment is uncomfortable enough to require numbing.

Best suited to: someone whose concerns include texture, acne scarring, or pore appearance alongside laxity, and who accepts downtime.

So Which One

The honest framing is not which device is best, but which layer your problem lives in.

If your skin looks crepey and dull with only slight looseness, the problem is in the dermis, and Sofwave or a skin quality approach addresses it. If your face has lost definition along the jaw and the tissue is descending, the problem is structural, and XERF or Ultherapy is the relevant category. If you have both laxity and texture damage, RF microneedling and similar combined treatments cover more ground than either. If your laxity is significant and you are looking at photographs of surgical results, no device here will get you there, and the useful conversation is a different one.

Between XERF and Ultherapy specifically, which is the comparison most people are actually making: Ultherapy goes deeper and has more published history. XERF treats multiple depths at once, engages the retaining ligaments, and is considerably more comfortable. For most people with mild to moderate laxity, comfort and tolerability matter more than they expect, because a treatment you dread is a treatment you do not repeat.

The Thing That Matters More Than the Device

Every device in this list can produce a poor result in inexperienced hands and a good one in skilled hands. Settings, depth selection, energy delivery, coverage discipline, and honest candidate selection influence outcomes more than the logo on the machine.

The provider who tells you a device will not do what you are asking for is worth more than the one who books you immediately.
